Insurance Information

Haverford College maintains a secondary insurance policy that aids in the payment of bills due to athletic injuries. To be eligible for this coverage, an injury must occur as a direct result of participation in varsity athletics. An injury that occurs outside of this settings is not eligible for consideration under this policy.  Injuries sustained prior to the athlete attending college and medical expenses incurred due to sickness or illness are also not covered by this policy.

Treatment that is sought outside of the knowledge of Haverford College's athletic training staff may not be eligible for coverage.

To file a secondary insurance claim, the student-athlete must present the following:

  • A completed claim form [PDF] Once the student-athlete's portion is completed and signed, please forward to Curt Mauger.
  • Itemized bills for all procedures that have remaining balances. (A balance due statement is not sufficient.)  These should be requested from the provider’s billing department.
  • Explanation of benefits or a letter of denial from the primary insurance provider for every remaining balance and date of service. (Claims that have not been processed by the primary insurance provider will not be considered.)
